Les tricycliques les plus fréquemment prescrits en France sont : Laroxyl® (amytriptyline), Anafranil® (clomipramine), Prothiadem® (dosulépine), Tofranil® (imipramine).

Dosages


The MULTIGENT Tricyclic Antidepressants assay is designed to detect a range of tricyclic antidepressant drugs and their metabolites. Expected values are difficult to define due to the range of cross-reactivity in this assay and varying concentrations at which these drugs may exert a biological effect.

Cardiac effects have been demonstrated with TCA blood levels as low as 50 to 100 ng/mL.

With levels greater than 500 ng/mL, the incidence of serious cardiac toxicity increases significantly.

The observed semiquantitative concentration must be interpreted in relation to the clinical presentation of the patient and, when possible, knowledge of the specific drug ingested.

Evolution


Patient asymptomatique et intoxication sup. à 6 heures : Surveillance en UHCD avec ECG à 12 heures puis à 24 heures de l’intoxication et évaluation psychiatrique.
